Scribes

I hate to call people tools, especially people who make my life so much easier, but still I think this is where they fit. I do not see how any busy ED manages to survive without using scribes, especially if they are anywhere near a college campus. These are generally pre-med students, highly motivated to enter the field of medicine. They are bright, intelligent, harworking folks who do not require a lot of money. Many would probably volunteer to have such an intimate glimpse and role in ED patient care. They want to please YOU the ED doc who may write them a letter of recommendation in a year or two.

They way I use them is as follows: I grab a chart glance at the CC, hand the scribe the chart and ask him or her to join me. I introduce myself and the scribe to the patient and then start talking to the patient. Everytime the patient says something I think is important I turn to the scribe and repeat or double check that this important piece of info has been documented. During the exam, i inform the scribe of every significant finding I note. Then at the end of the encounter I wash my hands as I dictate the orders and medications I would like for the patient. The scribe finishes that drops the orders off and we keep going.

Later in the shift (here is where the motivation part comes in) the scribe gathers lab, and imaging data, and acts as an accessory brain for me. For example, as I am dictating a chart or completing discharge paperwork, I may ask him or her to see what’s pending on the other patients and to find out what the hold up is. Unlike asking a nurse to do this, a scribe has no other duties. An IV, or foley, or hospital admission report will not sideline a scribe from fulfilling my needs. All of that for $10-30/hour, unbelievable! Even poor hospitals should use them, after all what costs more, a doctor chasing labs and imaging results, or a pre-med student?
